That's a tuff one.  Myself, I would be looking at my insurance policy and having a discussion with my agent first.  It could be that it is explicitly written in the contract that all passengers must wear a seatbelt.  Plus it's Ohio law.

We have recently had a conversation about seat belts for people we are serving... Our policy is that people should have their seatbelts on when we travel with them... Duh... These people are being transported in our company cars... Sometimes it is for NMT, sometimes it is just for OOD services where that transportation cost is built into the full fee to provide the service.

But, I'm thinking it would be a rights restriction if we refuse to transport them because they refuse to put their seatbelt on... I'm thinking a waiver or something would need to be signed saying they recognize the risk, but I thought I would bring it up to everyone to see what their thoughts are :)
