You may have received this survey from DODD. It's purpose is to assess how closely Ohio's adult day services settings align with the new HCBS regulations.  There has been a slow response so far.  We'd like to see a good response level from OPRA members. The survey and results will be part of the state's submission to CMS for the HCBS Transition Plan.

Please let us know if you have any questions about your response.  The deadline has been extended to October 31st.

Subject: survey reminder

This is just a reminder for all Vocational Habilitation and Adult Day Supports waiver provider agencies - both public and private - who serve individuals in congregate work and non-work settings, to please complete a separate self-assessment for each of the waiver settings where those services are being provided.  We recognize the amount of time needed to provide an accurate and comprehensive assessment for each location, and as such, have chosen to extend the due date for completion of the survey to close of business Friday, October 31st.  The survey can be accessed online by following this link: https://www.surveymonkey.com/s/HCBSAdultDaySettingSurvey.

We thank you in advance for your assistance on this matter, as it will provide the data we need to make a solid case for Ohio when we submit our Transition Plan to the Centers for Medicare and Medicaid Services (CMS) to show how we align with CMS' new HCBS Regulation.
