Hello All,
I am resending an e-mail I sent you on Nov. 2nd requesting feedback on the pending revisions to the Medication Self-Administration Assessment forms. If you would please confirm your receipt of this e-mail that will be helpful,
and note per instructions below we are
extending the deadline to 11/26.
Also included in this mail are new forms for nurses to assess G/J tube administrations, and insulin/injectable metabolic-glycemic medications.

Hello Stakeholders,
We are drafting revisions to the DODD Self-Administration Assessment forms, and instructions, to better align with current Category 1 Medication Administration rules and person-centered planning.
Please note that there are no substantive changes with regard to assessment criteria for a person to be considered able to self-administer, self-administer with assistance, or needing qualified support with medication administration.
The changes are merely to formatting/wording/instructions for clarity, consistency, and ease of use.
The versions attached here are DRAFTS. They have been developed with considerable nurse-stakeholder input, and now we want non-nurse-stakeholder input.
If you would like to help with this process, please do the following:
1-
Send this request to a small sample of SSAs and/or,
non-nurse providers to review the instructions and assessment forms. Please:
a.
Consider them in the context of individual’s that have already been assessed (please
do not use for actual assessments that will be part of ISP)
b.
Give feedback on
i. clarity of wording
ii. clarity of outcome determination

Have reviewers return their input directly to you; then please send all the feedback you get in a single e-mail to
ma.database@dodd.ohio.gov (with attachments of individual’s comments if you’d like to do that)
a.
Forms are open so the editorial features of adding comments to documents can be used
b.
General comments are welcome also

Once the forms are finalized by DODD they will be published early in 2019.
The intent at this point, is to have the new forms in effect for all NEW assessments by April 2018. Old forms may continue to be used for updating previous assessments for 2 years.
There will be no requirement for current assessments to be redone just to implement new forms. New forms will only need to be used for new assessments beginning April 1 2019.
