Hi Jennifer,

There are several instruments that we use in the Housing Navigator course;

-          Individualized Housing Options Resource Guide from Pacer center, in particular the brief plan on page 33. https://www.pacer.org/housing/gettingstarted/images/IHO-Resource-Guide.pdf

-          The YAI Assessment tool (attached)

-          The Life Skills Inventory from NY State OPWDD. (Attached)

These latter two are kind of formal and the language is not particularly person centered but they can help to create an holistic picture. Clearly when having a meeting with the individual the focus has to be on taking whatever time and preparation and multiple meetings are necessary to really get to understand the person.

-          Bob Laux Created “Your Place or Mine” from which I excerpted “getting along” (Attached) which gets to some of the realities.

In an effort to be more person centered we will be updating the tool we use for home alone assessments. We are looking for suggestions and sample tools. Many of the people we support do not use words to communicate. We need to design a tool that will meet the strengths and needs of all the people we support who live in the community. Thank you.
