I would suggest you contact The Arc, DODD's Family Council , and The DD Council to see if they can access or develop a assessment?  
I would also suggest you provide a dissenting opinion in the ISP suggesting you disagree and will monitor and provide the SSA with relevant data as it occurs. This may assist in partially shielding  you if a life threatening incident does occur if u accept doing the support plan.

We are being faced with SSA's who have are deciding that a few of the individuals being served no longer need supervision for periods of time.  We strongly disagree with the assessment, but are looking at this from a subjective perspective, as opposed to the financial perspective of the SSA (I suspect).  Does anyone have a good, user friendly, assessment that would look at the individual's skills in the areas of home and community safety.  The DDP does not really address this.  This would give us objective data to support our position that supervision continues to be needed, or let that we are being too protective.
