Hello,
Wanted to send out a reminder that the due date for the National Core Indicators Staff Stability Survey is quickly approaching (June 30, 2016). This survey will help the Department gather important information about Ohio's direct support
professional community. We would like to remind you that completing this survey is mandatory per DODD
Provider Certification rule 5123-2-2-01(D)(16). See page 9 in the rule attached;
(16) An agency provider shall participate as requested by the department in service delivery system data collection initiatives such as the national core indicators staff stability survey.
