Attached are the finalized CareStar options counseling materials. Thanks to everyone for all the feedback, much of which was incorporated into the final products.  The documents attached are as follows:

 

1.       Community Options Form – This is the form CareStar will use in conjunction with the individual sessions

2.       Comparison – this is the “main” document that will describe waiver and ICF differences/similarities

3.       Waiver matrix – this document provides high level information related to different waivers available for our population

4.       Easy read documents for what is a waiver, service plan, QIDP, and SSA

 

CareStar is working on scheduling both individual and general education sessions which are expected to begin as early as next week.
