Attached are some examples of the need for nursing care coordination for people with Waivers. I’m including everyone in this important issue to help clarify what care coordination is for those concerned.

In addition, take a look at the newly released 4 minute YouTube video released by AAIDD “Improving Health and Health Care for People with IDD”.  Susan Havercamp, the speaker, is a PhD with the Nisonger Center at Ohio State University.
