Subject: COVID19 Screening form

Thank you for all you are doing to support the providers during this difficult time.  Attached is an employee screening form the ICFs could use, if they choose.   I know that this is another layer and we want to make this as easy as possible for them.  The safety of the individuals and staff are a top priority.  Feel free to share the form if you feel it could be beneficial to the ICFs.  Also please ensure all providers have the DODD Support Teams information that came out yesterday.    Here is the link for the map  DODD Statewide Support Teams map.  Please feel free to contact me, if I don’t know the answer I will get you one!  Thanks.
