Hello –

I have attached a profile and have listed some information below.

We are currently seeking a H/PC provider for 2:1 staffing with a behavior add-on in a rural area, with or without roommates for a 25 year old young man, who is currently at Tiffin Developmental Center. 

The individuals father/guardian is interested in a placement somewhere closer to him. His guardian lived in  Kenton, OH which is why we are looking outside of Lucas County.  The team is open to exploring both ICF and waiver placement options.
