I am the new Program Director at First Christian Day Services in Canton and while the resigning Director was still here, together we attempted to complete the initial application with Rapback.  Since that time, I have received an email from them indicating the following message:   

Since this email notification, the medicaid # noted above has been confirmed as our provider # and I have attempted to fill out the application again, though I am still waiting for a response.

Any insight is greatly appreciated!
