It’s as simple as this under the new reform provisions: Medicare Advantage and Part D plans that earn four- and five-star quality ratings from CMS will have a significant competitive edge. Quality bonuses could total as much as 5% for MA plans beginning in 2012, and they will be based on actions and decisions that plans make this year and next. To further complicate an already tricky situation, CMS is considering major changes in what goes into the star ratings and the possibility of combining Part C and D ratings into one score.

The new head of CMS, Donald Berwick, M.D., is passionate about performance-based measurement. His outcomes orientation means the star-rating program will only get tougher, stressing tighter accountability and proven results. Since just a small percentage of plans now have the ratings of four stars and above needed to qualify for the new bonuses, MA sponsors will need to revamp their operations as soon as possible or risk losing significant Medicare revenues.

Before starting his company in 1996, Mr. Gorman served as assistant to the director of HCFA’s Office of Managed Care under two directors, providing both day-to-day management and external liaison for the Medicare and Medicaid managed care programs. Prior to that, he was staff director for U.S. Rep. John Conyers, Jr. (D), then chairman of the House Government Operations Committee.

FRANK INGARI is CEO of Essence Healthcare, a rapidly growing and successful MA plan operator based in Maryland Heights, Mo. Mr. Ingari has more than 30 years of experience as an entrepreneur and senior information technology executive. Prior to joining Essence, he was chairman and CEO of Shiva Corp., a pioneer in the remote-access market, and he took the company public with Goldman Sachs in 1994. Previously, he was vice president and general manager of the spreadsheet division at Lotus Development Corp.
