The dates and length of process are important. Since this is not done very often, you may want to discuss process with DODD as to who will hear the appeal after you go through local process? Also, role of county SSA is represent the consumer, not the provider. Have your data on why you are giving the notice backed up with information on what you have tried to do to make it a successful admittance. If it is around behavior challenges, provide the BSP and attempts to implement it. If it is medical condition or ambulation change indicate environmental prohibition to the housing situation. We also try to offer alternative placement locations or a plan that could be successful, if funded.
